Volunteer Clearances

Volunteers are an integral part of the Dallas School District team, and the district appreciates the student opportunities made possible by your donation of time.

In compliance with state guidelines, the district requires all volunteers submit background clearances and receive Board approval.

If you have been a Pennsylvania resident for the past ten (10) years and have not been convicted of enumerated offenses, you may submit this Affidavit, in lieu of the FBI fingerprinting check.

Volunteer Affirmation Form

State Police Clearance

You can apply online here, and you will receive results immediately. Print and retain the receipt containing the control number, which starts with the letter “R.” The cost of this clearance is $22, payable by credit card on the website.

You can also fill out, print, and send this form directly to the Pennsylvania State Police Central Repository-164. Click here for the form for employees and here for volunteers. You will need to include a $22 money order with your form (retain your money order receipt until you receive your official clearance). This clearance will be mailed back to the address you provide on the form, and takes, at a minimum, four weeks to be returned.

Employers will need to see the original and retain a verified copy.

Child Abuse Clearance

The cost of this clearance is $13 paid by credit card on the website when you apply here. This clearance takes, at a minimum, two to three weeks to be returned.

To schedule your 10-minute school volunteer fingerprinting appointment, enter the following service code: 1KG6ZJ online. You can also call 1-844-321-2101 to schedule your appointment. The cost of this clearance is $23.85, payable at the time of your fingerprinting appointment.

Employers will need an UEID number or your full Social Security Number to run a report.
