For families with students currently enrolled in the Dallas School District, click on Online Registration, login to your Parent Portal account, and click “Register New Student” on the center right of your portal home screen.

If you do not have a Parent Portal Account or cannot remember your username and password, click on Online Registration, and then click on the “Online Registration” link, which is located below the Parent Portal login.

For new families to the Dallas School District, click on Online Registration, and then click on the “Online Registration” link, which is located below the Parent Portal login.

Use Google Chrome for best results. Apple devices will not work. You must submit the form within 60 minutes or the form will timeout due to inactivity.

Completing Your Kindergarten Registration

All students enrolling in Kindergarten must be 5 years or older before September 1, 2022.

After completing the CSIU online registration process, please call 570.674.7255 to make an appointment to complete the registration process.

You will need to bring these completed forms and supporting documentation to your appointment:

  • Kindergarten Medical Form
  • Kindergarten Transportation Form
  • Required Supporting Documentation:
    • Two Proofs of Residence
    • Birth Certificate
    • Driver’s License
    • Student Immunizations

Please Note: The Weapons Affidavit form MUST be notarized. Do NOT fill this form out until you are in the presence of a notary. The District provides notary services, free of charge, for registering families. Please contact Kelly St. Clair at 570.674.7255 to set up an appointment.

Instructions for Completing Fillable PDF Forms

To electronically sign all documents:

  • Click on “Fill and Sign” in Adobe
  • Create your signature
  • Place your signature on each form on the necessary signature lines
  • Save the fillable PDF by using your child’s name (ex. Smith_John.pdf)
  • Scan or take photos of required documents
    • Parents’ Photo IDs (will be used to verify identity)
    • Two Proofs of Residence (ex: deed; lease/mortgage agreement; recent home utility bill such as gas, electric, water, cable, internet, cell phone; vehicle insurance; current check stubs from wages, public assistance, social security, or other source of income; property/school tax statement; current bank statement)
    • Student’s Birth Certificate
    • Student’s Immunization Records
    • Custody Agreements (if applicable)
  • Send all saved PDFs and scanned documents to registration@dsdhs.com. Please make sure to reference student’s full name and parent name in the subject of the email.
